Staff 

1. Please click the ‘Click to download’ button. 

2. You will be directed to Microsoft Sign In. Please sign in using USM email and password. 

3. The form will appear in a new tab and ready to be downloaded. 

Student 

1. Please click the ‘Click to download’ button. 

2. You will be directed to Microsoft Sign In. Please sign in using USM email and password. 

3. Upon sign in, a notification email will be sent to USM IACUC Secretariat. The Secretariat will approve your request to access the form within 24 hours.  

4. Within 24 hours, you will be able to download the form by repeating steps 1-2. 

5. The form will appear in a new tab and ready to be downloaded.
